Dear Doctor,
I have developed a significant change in my sexual response. I now become strongly erect very quickly, sometimes within 3–5 seconds, even from mild sexual stimulation such as seeing, reading, remembering or thinking about something sexual. More recently, erections take about 5–10 seconds and gradually become firm.During arousal, I experience fullness, congestion and pressure in my testicles, perineum and pelvic area, together with tension or spasm in my pelvic floor muscles. These symptoms improve after ejaculation but return with renewed arousal. After ejaculation, the erection can sometimes remain strong enough for a second sexual round.
Before these symptoms developed, I practiced Kegel exercises excessively and trained myself to maintain very strong erections for as long as possible. I used sexual imagination while lying on my back and tightened my abdominal, leg and pelvic floor muscles. I also practiced edging and combined it with Kegels, even trying to maintain an erection during physical exercises such as push-ups and jumping.
On one occasion, I maintained an extremely strong erection for about 113 seconds, with very intense internal pressure in the penis. During prolonged intercourse, I sometimes experienced penile numbness while the penis remained strongly erect.
My GP referred me for pelvic floor treatment. I stopped the exercises and attended three sessions, but could not continue because of the cost. Deep breathing and relaxation reduced the intensity somewhat, but the symptoms remain.
I have not had sexual intercourse for about 12 months. After approximately 7–10 days without ejaculation, the fullness and congestion in my testicles and pelvic area become uncomfortable, and ejaculation relieves them temporarily.
Could excessive Kegel exercises, edging and prolonged erection training have caused pelvic floor overactivity or spasm? Could there be another urological, prostate, neurological or vascular cause? What examination or tests would you recommend?
Thank you for your advice.
Kind regards,
